STEP #2: Complete this brief form (takes only 10 minutes).

STEP #3: Give the form to your SPONSOR (he/she will do all the rest).

STEP #4: Come to the “Great Banquet Weekend” with your sponsor!

** NOTE ** There is no charge for the weekend, however, a $10.00 registration fee (payable to HGB) is requested. Financial assistance is available if needed.
